Memorial Hospice is proud to be the charitable beneficiary of the Polo Classic. Serving Sonoma County for the past nine years, Memorial Hospice is a non-profit program of the St. Joseph Health System – Sonoma County, a ministry of the Sisters of St. Joseph of Orange. Memorial Hospice provides compassionate hospice care and grief support services to individuals and families facing a life-threatening illness or death of a loved one.



Our work is priceless and offered to all who need us

Demand for our services continues to grow as more patients and families realize the benefits of Hospice care and our extensive grief support services. Together with our sister program Hospice of Petaluma, we care for an average of over 3,300 patients and family members annualy. Our Volunteer Services, Grief Services, Community Education and care for uninsured individuals are supported by donations and fundraising.

Memorial Hospice

The staff and volunteers of Memorial Hospice are committed to providing the best care – physically, emotionally, spiritually – to those in our community. Your support provides:
• Compassionate and comprehensive medical home care for people nearing the end of life,
• Spiritual and Social Services support for patients and their family members,
• Grief support services for children, teens and adults – for anyone in our community,
• Training for caregiver and grief support volunteers,
• Community education to provide awareness about end of life questions and concerns, and
• Hospice care and grief support services for the Latino community.

Memorial Hospice was founded in Santa Rosa in the year 1997. Its headquarters are located in a Spanish-style home on Mendocino Avenue. Memorial Hospice is associated with the Hospice of Petaluma in Petaluma. Both of these hospices offer the best hospice care and grief support services to residents throughout Sonoma County, and both are operated under the St. Joeseph Health System. There is no charge to the patients, nor their families, nor the grief support clients for the service they receive.

Revenue is raised through general donations, the sales at the two Sack's thrift stores, and through fund-raising events such as the POLO CLASSIC. These funds help to provide care for uninsured individuals, Volunteer Services, Grief Services, and Hospice's Community Education Program.
